Employee Demographic Data Changes and Human Resource Self-Service Module

Posted:

New York State recently enacted “Collection of Demographic Information (Executive (EXC) Chapter 18, Article 7, Section 170-E*2)” reporting requirements along with Executive Order No. 187, expanding demographic self-identification options. The Human Resource Management Office invites all employees to review their records and voluntarily make changes.

Employees may update their personal phone numbers, home addresses, and emergency contacts and view their legal name, demographic, and education information in the SUNY System Administration’s Human Resources Self-Service Module.

Employees are encouraged to review their records annually to ensure that their information is accurate and up to date. Necessary changes may be completed or requested via the Self-Service Module. A user guide (PDF, 364 KB) is available to assist employees in accessing and using the Self-Service Module.

Home address and legal name changes may also be requested by completing the paper form. Campus directory updates, including campus address and phone extension, may be requested using the online Directory Add/Update Form. Departments with multiple directory edits for their staff members may email a list of the changes to Erika Grande, manager of personnel operations, in Human Resource Management.

Please email benefits@buffalostate.edu with questions.

Withdrawal from Course: Change to Deadline

Posted:

The deadline for Withdrawal from Course will align with the pass/fail application deadline starting with the spring 2024 semester. The spring 2024 deadline for course withdrawals and for pass/fail applications is Friday, May 3. The fall 2024 deadline for course withdrawals and for pass/fail applications is Friday, November 22.

Withdrawal from Course Reminders
Instructors are required to notify students of their academic standing no later than the end of the ninth week of the semester, or after two-thirds of a summer session or January Term. If a student does not officially withdraw from a course and stops attending, a grade of E will be recorded.

Students who wish to leave a course after the drop/add period and before the final withdrawal date may withdraw via Self-Service Banner. Nonattendance at the first class meeting does not withdraw a student from a class.

Withdrawals remain on the transcript and are recorded as Ws but do not count in credit hours or GPAs. Withdrawal from a course may affect eligibility for financial aid in the current semester and in future semesters.

Students are asked to give careful thought before withdrawing from a course, as course withdrawals can have a negative impact on both their progress toward graduation and eligibility for financial aid. Withdrawal from Course should be used infrequently and considered an action of last resort. It is to the student’s advantage to read the instructions under Withdrawal from Course before withdrawing.

Important dates are published in the Academic Calendar for each term.

Grading Reminder: Last Date of Attendance

Posted:

When issuing a student a non-passing grade, including an E, F, U, I, or N grade, please also report the student's last date of attendance when entering the grade online. This assists the Financial Aid Office in determining whether a student unofficially withdrew and whether the student is able to retain the aid he or she received.

The last date of attendance is the last date the student participated in class, took an exam, handed in homework, or actively engaged in an online course. If assigning an I or N grade, the last date of attendance is the last date you met with the student to discuss progress in the course. If you have no record of the student's attending, do not enter a date. Instead, use the grade of EV, which indicates that the student never attended the course.

Questions regarding this policy or procedure should be referred to the Financial Aid Office, (716) 878-4902. The grading guide (PDF, 162 KB) may also assist you.

2023 SEFA Campaign: 'Why We Give' Segment

Posted:

“Donating to the United Way SEFA Campaign is a great way to make a positive impact. SEFA provides support to organizations that help people in need. It also brings people together to address issues that governments and businesses may need help to solve. Even small donations can make a difference. Show your support for the causes you care about and help these organizations create a positive impact on people's lives.”
—Ken Fujiuchi

We warmly invite you to share your reasons for giving, and we'll feature them in the Daily until the end of the SEFA campaign on December 14. Please send your thoughts to campaign co-chairs Wendy Paterson, dean of the School of Education, or Maria C. Garrity, senior systems programmer in Information Technology. Let's maintain our momentum, as together, we can achieve this! To donate, please go to the e-pledge system and click "Forgot user ID or password" to begin the process, even if you are rolling over your pledge from last year because it is not automatic. This instructional video and documentation can further guide you. If you prefer to donate by cash or check, please complete the paper form (PDF, 4 MB) and submit it to the School of Education, Bacon Hall 306, or to your departmental ambassador. The list of charitable agencies (PDF, 10 MB) is available online. Thank you.
