Annual report forms are posted on the web at www.buffalostate.edu/offices/hr/forms.asp. The timetable for submission of annual reports for 2011–2012 follows:
June 11, 2012: Individual reports from faculty, librarians, and professional staff submitted to director or department chair.
July 9, 2012: Department reports to dean, associate vice president, or vice president.
July 27, 2012: All reports (deans, associate vice presidents, vice presidents) to provost (submitted electronically to Carolyn Martino.
