Employees who enrolled in the 2009 Flex Spending Account (FSA) Program should have received a confirmation letter from the Fringe Benefits Management Company. Please note: Corrections to your 2009 FSA enrollment and/or changes to your annual contribution must be made in writing on the confirmation letter and faxed to (850) 514-5804 no later than 5:00 p.m., Wednesday, December 10, 2008.
