An Excel version of the Purchase Requisition is now available on the Procurement Services website under “Forms." This form will calculate the total price for you. Please fill out the website version of the form instead of any version you may have saved on your computer to ensure that you are using the most current version. Any future updates to the form will be noted on the website.
