THIS CLINIC HAS BEEN CANCELED. VACCINE UNAVAILABLE.
Seasonal flu shots will be given in the Campbell Student Union Assembly Hall on Tuesday, October 20, from 10:00 a.m. to 1:00 p.m. No appointment is necessary.
The vaccine being administered protects against seasonal influenza. This is not the H1N1 (swine flu) vaccine
The following insurances will be accepted: Community Blue, Independent Health, Univera, Blue Choice, and Medicare Part B. Employees must show valid insurance ID cards. Employees without proper insurance coverage will be charged $30 for the flu shot.
Empire Plan enrollees may obtain a flu shot from a participating provider’s (doctor’s) office for their usual co-pay if they wish to do so.
If you have any questions regarding your health insurance coverage, please call Employee Benefits at 878-4821 or the member services division of your insurance company. The phone number should be on your insurance card.
